Imaging Referrals ​

Accepted electronically when submitting the CBCT referral form. By completing and submitting our imaging referral form, you agree to the terms conditions set out in this service level agreement.

1. Purpose of This Agreement

 

This agreement governs referrals to one80 Dental for CBCT imaging and a radiology report by Dr Deleep Basi. It ensures compliance with IR(ME)R 2017 (and amendments), GDC Standards, UK GDPR / Data Protection Act 2018, and national dental radiography guidance.

2. Responsibilities of the Referring Clinician

You confirm you are IR(ME)R-entitled as a Referrer and have relevant radiography training. The CBCT scan is clinically justified, and alternative imaging has been considered. You have supplied adequate information for justification:

• Clinical question & region of interest

• Relevant medical history (including pregnancy status if applicable)

• Previous radiographs or CBCT (dates)

• You have obtained and documented the patient’s informed consent for CBCT and data sharing.

• You will transmit data securely (encrypted email or secure upload).

• You will act on the CBCT report once received.

3. Responsibilities of one80 Dental / Dr Deleep Basi

​

All CBCT imaging is carried out under IR(ME)R Employer’s Procedures with entitled Practitioners and Operators. Every CBCT exposure is justified and authorised before imaging. CBCT scans are performed using ALARP principles (dose optimisation). A formal radiology report is provided by an appropriately trained clinician.

Typical turnaround:

 

• Appointment offered: within 5 working days

• Report issued: within 10 working days after the scan

• Urgent or unexpected findings will be communicated directly to the Referrer.

• All data is securely stored and processed under UK GDPR.

• Accidental or unintended exposures will be managed and reported to the CQC where required.

Your referral must include:

​

• Full patient identifiers

• Referrer name, GDC number, practice details

• Clinical indication and requested field of view (FOV)

• Relevant radiographs or clinical photos

• Medical history including red flags

• Referrals may be queried or declined if insufficient for IR(ME)R justification.

5. Fees & Patient Information

​

Fees for CBCT and reporting are set by one80 Dental. The Referrer is responsible for ensuring the patient is informed of any charges unless otherwise agreed.

6. Complaints & Communication

 

Any concerns relating to imaging or reporting should be directed to one80 Dental.  Where a patient complaint involves both practices, a coordinated response will be provided.

 

7. Agreement & Electronic Acceptance

​

By submitting the CBCT referral form, you confirm that:

​

• You have read and agree to this SLA.

• You meet IR(ME)R requirements as the Referrer.

• You have obtained valid, informed consent from the patient.

• All information supplied is true and complete.
